Investigation Summary
At approximately 11:15 a.m. on April 26, 1994, Employees #1 and #2 were elevating a Marklift 100 aerial lift after stocking their paint supplies. The operator raised the aerial lift above the angle the machine limit switch would allow and extended the 100 ft boom to its maximum length. When the operator began to lower the basket and swing the lift toward a two-story building, the lift tipped over. The two employees were seriously injured: Employee #1 died of massive head injuries on April 30, 1994. The investigation revealed that, although the boom had been fully extended before reaching the required 55 degree angle, the employees had been operating the machine within the guidelines established by the manufacturer. The accident was the result of equipment failure. The limit switches on the lift did not prevent the boom from extending too far before reaching the required 55 degree angle.
