Employee falls 13 feet and sustains fractures and a puncture

At 2:15 p.m. on May 4, 2017, Employee #1 was setting the grading for a walkway, at a height of 100 feet, in a structure under construction. The employee fell 13 feet to the deck below, hitting a lower beam on his way down. Employee #1 was wearing fall protection, a Guardian 2000, but the I-beam clamp that was used as an anchor failed. It appears that the carabiner bound against the ratchet release on the I-beam clamp during the fall, releasing its grip on the I-beam. The employee stated that he felt the lanyard jerk as if it was going to break his fall, then he felt it release and he fell all the way to the deck below. Employee #1 sustained two broken arms, broken ribs and a punctured lung.
