Investigation Summary
Employee #1 was a batch operator in a cement mixing plant. He finished loading a cement truck full of cement and told the truck driver that he needed some dye to mix with the cement. The truck driver drove out from under the cement hopper and was going to get dye from a shed in front of the batch building, when he saw Employee #1 lying on the rear left rubber tire of a front end loader adjacent to the batch building. He got help, but Employee #1 died, apparently of a heart attack.
