Employee crushed when struck by back hoe bucket

Employee #1 and another worker were working on a section of pipe approximately ten inches round and three feet long. They were trying to fit this pipe to another pipe in a trench that was dug earlier. There wasn't enough room in the trench and they decided to bring the pipe back out for assembly. The pipe was placed on the bucket of a back hoe when the bucket swung fast and pinned Employee #1 against the wall. Employee #1 died.
