Event: 03/17/1995
Employee dies in fall from structural steel I-beam

At approximately 10:30 a.m. on March 17, 1995, Employee #1 was on top of an I-beam spreading and connecting bar joists. A bundle of bar joists had just been set on the I-beams by a forklift. The operator put the forklift in reverse but realized he was going to run over some materials with his rear tires. He put the forklift in forward but did not lower the mast. The mast came in contact with the bundle of bar joists, knocking it off the beams. Employee #1 either was knocked off by the bar joists or moved to get away and lost his balance. The employee, who was not tied off, fell approximately 18 ft to a concrete slab. The employee died of a ruptured pulmonary vein.
