Investigation Summary
At approximately 5:00 p.m. on March 11, 1995, Employee #1 was helping another employee put plastic over roofing materials that were stored on a roof. A gust of wind caught the plastic and Employee #1 backed up to straighten it out. He backed into an unguarded 4 ft by 4 ft skylight hole and fell approximately 21 ft to a concrete floor. Employee #1 died.
