Investigation Summary
At 3:15 p.m. on April 26, 2017, an employee was using a handheld sponge to clean a circular metal disk, model number MVP20MP, serial number 1013, that is used for filling cups with rice pudding. The employee caught his left middle finger when he attempted to clean the disk when it paused during operation and amputated the first joint from the employee's finger. Hospitalization was not required.
