Investigation Summary
Employee #1, age 20, was climbing atop a trailer to open a pneumatic valve in preparation for filling the trailer with fertilizer. The plant manager, who was sitting in the cab of the truck, said that the bin, the supporting structure, and the conveyor began to shake and then fell. Employee #1 was killed when he was crushed between the bin and the trailer.
