Investigation Summary
Employee #1 was replacing air bags on the rear right side of a bus and had jacked up the frame on the right side with a 10-ton jack. He then placed another cylinder jack rated at 5 tons between the frame and the body of the bus to make room for the new air bags. The old air bags had been removed when a coworker cleaning the inside the bus felt it suddenly drop and then heard a moan. He went outside and found Employee #1 draped over a tire, with the frame of the bus on his back. Most of Employee #1's ribs were broken and his chest crushed; he was killed. The 5-ton jack had slipped out, causing the bus to fall. Jack stands and other safety equipment were not being used. Employee #1 was an experienced bus mechanic with 23 years of experience performing bus maintenance.
