Employee dies after fall through skylight

At approximately 11:00 a.m. on February 24, 1994, Employee #1 was working with a foreman of the company on the roof of a building at Boland Marine and Mfg. Co., New Orleans, LA. He was helping the foreman install sheet metal on the edge of the building. The roof had approximately 26 skylight openings that were covered with fiberglass panels measuring approximately 10 ft long by 3 ft wide. The roof was made of corrugated metal and the fiberglass panels also had a corrugated appearance. The panels were distinguishable from the surrounding metal by their whiter opaque color. Employee #1 accidentally stepped onto one of these fiberglass panels and fell about 25 ft, suffering injuries from which he died.
