Investigation Summary
At approximately 9:05 on July 16, 1992, Employee #1, of Ted Heine Construction, was helping the owner survey the grade of a trench that had been opened the previous day. The owner had not trained Employee #1 in working around a trench, even though he was working outside his regularly assigned job as a heavy equipment mechanic; however, Employee #1 had helped survey during construction of a dirt dam. Employee #1 walked approximately 10 ft into the trench. He was killed when the trench caved in, and was declared dead at the job site. The owner stated that he was unaware that Employee #1 was in the trench when the cave-in occurred.
