Investigation Summary
At 6:57 a.m. on May 17, 2017, an employee was adjusting/tightening the guide tube on a Modern Cut Off Machine, when his left index finger became caught in the machine's moving belt. This created a nip point by was of the belt and the pulley. As a result the tip of his left index finger was amputated. The incident investigation revealed that no lockout/tagout procedures were administered before servicing of the machine was initiated.
