Employee Is Caught Under Overturned Forklift and Later Dies

On June 29, 2006, Employee #1 was using a Hyster forklift with a clamp attachment to pick up and place appliances and boxes into a dumpster. Employee #1 was operating the forklift with the clamp in the raised position (approximately 128 in. above the ground) and was not wearing the seat belt. When Employee #1 made a sharp turn, the forklift started to turn over. Employee #1 was thrown out and the forklift fell on top of him. Employee #1 suffered unspecified fractures and died on July 2, 2006, from the injuries he sustained.
