Investigation Summary
On February 20, 2002, Employee #1 was part of a crew working on the final stages of a new sanitary sewer system. In preparation for environmental testing, water needed to be pumped out of the bottom of a manhole within the system. A coworker borrowed a gasoline-powered pump from another employer to remove the water. The hose for the pump did not reach to the bottom, so the employees lowered the pump down the manhole and secured it to a ladder with a rope. A short while later, the pump stopped running. Employee #1 climbed down the ladder and was overcome by exposure to carbon monoxide. Employee #2 climbed down to rescue Employee #1 and was also overcome. Employee #1 died as a result of the exposure and Employee #2 was hospitalized for carbon monoxide poisoning.
