Investigation Summary
On June 29, 2002, a work crew was flaring flammable vapors off of a rail tank car containing degradation products of 1,3-butadiene. Water was used to displace the vapors from the interior space of the car. However, the vapors had not been sufficiently removed. Employee #1 began cutting a hole in a side wall of the tank to drain the water and mark the tank "as safe" for salvaging operations. As Employee #1 continued to cut the tank, water drained out to the level of the cutting torch. The flame ignited the flammable vapors in the tank causing an explosion. Most of the water was expelled out of the tank from the force of the explosion. Employee #1 was hurled over a 4-foot livestock fence and landed about 48 feet away from the tank. Employees #2 and #3, who were between the tank and fence, were thrown into the fence. Employees #4 and #5, who were outside the fence line, were washed across a road into another fence. Employee #1 died from injuries about two weeks after the accident. The other employees had bruises. It is not clear whether the other employees went to a local hospital for treatment.
