Investigation Summary
At 3:00 p.m. on May 11, 2017, Employee #1, employed by a stone and tile company, was operating a table saw. He was cutting backer board for a mosaic when the backer board kicked back. Employee #1's left thumb came into contact with the table saw blade. Emergency services were called, and Employee #1 was transported to the hospital.
