Investigation Summary

Investigation Nr: 200810034
Event: 04/28/1996
Employee killed when drip gas tanker truck ignites

Employee #1 was loading his tanker with drip gas, a natural gas byproduct. After completing the process, he went on top of the tanker and opened the hatch to verify that the tanker was full. As Employee #1 released the hatch, liquid vapors began to escape and were blown toward the diesel tractor. The vapors were sucked into the tractor's air intake, causing the diesel engine to begin racing into a runaway condition. The engine exploded, and the resulting fire ignited the remaining fuel in the truck's fuel tank and the drip gas in the tanker itself. Employee #1 was killed.
