Investigation Summary
At 6:45 p.m. on May 12, 2017, an employee was indexing the turret carrier plate for the Turret Rewind machine. The employee was attempting to remove a label from the turret carrier plate while it was indexing clockwise and his right hand was caught between the carrier plate and the frame of the Turret Rewind machine. The employee received muscle and soft tissue trauma to the right hand, wrist and forearm.
