Employee crushed to death by falling machine

Employee #1 attempted to stop a fertilizer lump buster machine from falling off the forks of a forklift by standing in front of it in order to hold it back. The top-heavy machine, which weighed approximately 1,500 lb, fell onto the employee, crushing him. He died instantly.
