Employee crushed to death between lift and truck bed

Employee #1 was trying to operate a Tommy lift on the back of a 1985 Chevrolet truck when he noticed a spark in some wires under the back of the truck near the lift. He crawled over the top of the lift, which was down approximately 18 to 20 inches from the truck's bed, to reach the wires. When he touched them, the lift started and caught him between it and the truck bed. The employee's chest was crushed. He died instantly.
