Employee killed when caught between forklift and trailer bed

Employee #1 was the driver of a truck that had been repositioned after loading. The truck had been repositioned so that a forklift could use its side-shift forks to move the load to prevent it from extending over the trailer bed. Employee #1 was seen on the opposite side of the truck from the forklift, tossing a tie-down strap over the load. The forklift operator shifted the load and started to move forward. The forklift was heading parallel to the trailer, but the position of the rear wheels carried it toward the trailer bed. After a short distance, the operator felt a bump and stopped. He saw Employee #1 lying between the trailer bed and the forklift. Employee #1 died from his injuries.
