Investigation Summary
At 10:45 a.m. on May 8, 2017, an employee was cutting a 3-in. by 4-in. rectangular metal tubing into 11 inch parts using an semi automated Amada Horizontal band saw. The tubing was in 21 foot long sections. The employee tried to reach in to grab a off cut piece without stopping the machine and his finger came in contact with the energized blade casing injury to his right index finger. The employees was hospitalized where his right hand index finger had to be amputated to the 1st joint as a result of the injury.
