Investigation Summary
On September 1, 2003, Employees # 1, #2, and #3 built two wood carpenter bracket scaffolds. When they stepped out onto the northeast scaffold, the center support failed. Employees #1. #2 and #3 fell to the ground. Employees #1 and #2 were transported to Greenwich Hospital and Employee #3 was transported to Stamford Hospital. Employees #1 and #2 were treated and released and Employee #3 was hospitalized at Greenwich Hospital for 4-5 days and treated for fracture.
