Investigation Summary
This company has two work shifts - 7:00 a.m. to 3:30 p.m. and 3:30 p.m. to 11:30p.m. At approximately 11:30 a.m. on May 22, 2001, a first shift employee loaded one half-full cardboard barrel of aluminum metal parts (Part Number 1221) into tumbler compartment Number 53. Employee #1, who is assigned to the tumbler area on the second shift, also put rocks and water in with the parts until the compartment was about half full. The tumbler was started. The first shift employee left at approximately 4:30 p.m. and informed his supervisor that the parts would be finished during the second shift. The supervisor, who works first shift, was working later this evening due to working at Magnetic Particle Inspection, which is his station. The supervisor instructed Employee #1, who works the second shift, at approximately 5:30 p.m. to take the parts out of compartment Number 53 and put in the parts they were grinding in the grinding room in the same tumbler. The supervisor then informed Employee #1 to take those parts out before he left. The supervisor was at his station when he heard a load bang. Another employee assigned to work in the tumbling area with Employee #1 was in the dryer room adjacent to the tumbling area and did not witness the incident. Upon hearing the load bang, the coworker in the dryer room looked and observed Employee #1 getting up off the floor. The coworker assisted Employee #1 and walked him over to the loading dock area. The supervisor observed Employee #1 leaning over one of the steel 55-gallon container drums. Employee #1 was transported to the hospital where he died.
