Investigation Summary
At 1:00 p.m. on May 10, 2017, Employee #1 and Coworker #1, employed by a construction company, were engaged in exterior carpentry at a commercial building construction site. Coworker #1 was about to disconnect rigging straps that held an exterior wood-framed wall to a telescoping boom forklift. Employee #1, the lift operator, thinking the rigging straps were disconnected, backed up the lift with the straps still connected to the wall. The wall tipped over, knocking over the forklift with Employee #1 inside. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for a fractured pelvis and fractured heel.
