Investigation Summary
At 9:45 a.m. on May 10, 2017, an employee was clearing a jammed flattened cardboard box from the counter-ejector area of the Flexo 5176 machine. The employee had locked-out the machine in accordance with the company's procedures. While the employee was pulling the box out of the counter-ejector, a coworker operated the control for the counter-ejector (which was not de-energized because the employer's lock-out/tag-out "IES" procedures allowed for continued power supply to the momentary contact switch that controlled the counter ejector adjustment function). The employee's gloved left hand was caught on a screw or a projection on the shaft which is part of the adjustment function of the counter-ejector. As the shaft rotated clockwise, the employee's hand was wrapped up in the rotation which pulled the employee's left arm and was amputated at the elbow.
