Investigation Summary
On December 27, 2005, Employee #1 and coworkers were working on a patio, installing the exterior plywood on the north side of the main portion of a building. The workers were divided into two crews. One crew was inside, cutting and handing out the plywood through the open, metal-framed exterior wall; the second crew was outside, using electric screw guns to secure the plywood to the metal wall studs. The interior crew was working on the second-floor perimeter balcony while the exterior crew was working on the separated ladder sections. Employee #1 was securing plywood while standing on the fly section of the ladder when the bottom of the ladder slid out from the wall and he fell onto the concrete patio floor. Emergency services was notified and he was transported to Stamford Hospital, where he was admitted for treatment.
