Investigation Summary
At 8:45 p.m. on May 3, 2017, Employee #1 was asked by a coworker to help troubleshoot a glue build up on the Elliott case sealer on line 375. The employee inspected the machine, then laid down on the floor next to the machine. Employee #1 stuck his arm up into the machine to try to get some of the glue that was building up. The machine was still running and the chain bar mechanism caught the employee's arm, popping the arm out of its socket. The employee sustained a dislocated shoulder and was hospitalized for his injury.
