Investigation Summary
At approximately 7:00 p.m. on April 26, 2017, an employee was observing and directing a recently assembled steel A-frame hoist structure being pushed into position with the forks on a forklift. The assembled steel hoist frame/legs which were not anchored to the floor and being pushed into position fell over due to a spring like action of the support legs. The main steel support leg struck the employee's right foot, fracturing it. The small toe on the right foot later became infected and was amputated. The employee was hospitalized and treated for his injuries.
