Investigation Summary
At approximately 3:45 a.m. on April 27, 2017, an employee was changing a welding tip on the welder in cell 152. Employee had just completed a tip change in the cell. She stepped back and turned to step away from the welder. When she stepped forward she caught her foot on the yellow guard secured to the floor. This caused her to trip and fall forward. She struck her upper right arm and shoulder on the frame of the door to the cell. The employee fractured the top of the right humerus and dislocated at the right shoulder. She was hospitalized and treated for her injuries.
