Investigation Summary
At 4:10 p.m. on April 25, 2017, an employee was working at a 80 ton press when a piece of material became jammed. The press was shut off prior to the employee trying to disengage the material. The feeder was not shut off and continued to feed the material. The employee's right index finger was caught on the feeder and his first knuckle was amputated.
