Investigation Summary
At approximately 3:30 p.m. on April 4, 2017, Employee #1 was using a horizontal bandsaw to cut steel bolts. While cutting the bolts, the material moved and needed readjustment. Employee #1 did not stop the saw, and tried to move the bolts while the machine was running. His left hand was struck by the saw blade, breaking four fingers and severely lacerating his entire hand. A supervisor transported him to the emergency room of a medical center, where he received treatment and was then hospitalized.
