Investigation Summary
At 11:30 p.m. on March 16, 2017, an employee was using an electric double pallet jack while removing an empty pallet. The employee backed the jack up and did not notice the rack behind him. The employee hit the rack and pinned himself between the rack and pallet jack control. The employee suffered a laceration and needed surgery to his right hand.
