Investigation Summary
At approximately 11:30 a.m. on April 4, 2017, Employee #1 was working on elevated platform 25 inches above adjacent floor. He slipped and fell from the platform, landing onto the floor and machine components, including a tube tool stand. Employee #1 received contusion on his lower backside torso in the fall. Employee #1 was transported to a medical center, where he received treatment and was then released.
