At approximately 11:15 p.m. on April 6, 2017, Employee #1 was removing tails from packaged product. He reached into a OSSID packaging machine (Number 9) without the machine being locked out or tagged out. The packaging machine caught him and caused an unspecified amputation. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized.
