Investigation Summary
At 10:30 a.m. on March 29, 2017, Employee #1 and Employee #2 were removing bags of cement that had piled at the top of a conveyor. Employee #1 was on a ladder, slipped and almost fell from the ladder, lacerating his right pinky finger. Employee #2 told Employee #1 to have his injury taken care of and began climbing the ladder to replace Employee #1. Employee #2 was removing the last bag from the conveyor and caught his left hand and pulled it between the band and rollers. A coworker shut down the machinery and had to cut the belt for Employee #2 to remove his hand. Employee #2 was admitted to the hospital for surgery and skin grafting to replace the skin that was pulled off the top of his right hand. Employee #1 was not hospitalized.
