Investigation Summary
At approximately 6:45 p.m. on May 7, 1996, Employee #1 was working in a 14 ft wide trench that was between 9 and 11 ft deep. There was a trench box in place, but he was working approximately 1 ft outside of it, filling lift holes in the manhole sections with cement. As he was working, a trench wall caved in, burying Employee #1. He was killed. The cave-in was not immediately noticed by his coworkers, who erected a fence nearby after it had occurred and failed to see anything amiss. Employee #1 was found approximately 30 minutes after that, when a search was conducted.
