Investigation Summary

Investigation Nr: 360967897
Event: 09/30/1996
Employee injured by falling load of trusses, later dies

Employee #1 was contracted to a site employer as a hauler. He and a coworker were standing on top of his flatbed truck as a second coworker attempted to place a load of banded 26 ft roof trusses onto the floor near a load of smaller roof trusses that were already secured in place on the truck. The forklift operator could not adequately clear the flatbed load with the 26 ft trusses, so Employee #1 and his coworker attempted to hold them while the driver backed out to adjust his position. The driver released the load-engaging means prior to backing out. As he did so, the load tipped forward and struck Employee #1. He fell backward off the truck, striking his head on the concrete pavement before the load of trusses fell on him. His coworker on the truck was able to jump off as the trusses were falling. Employee #1 died on October 5, 1996, as a result of his injuries.
