Employee killed when caught between dumpster and chain

Employee #1 was unloading cardboard from a 2 yd dumpster and loading the material into a 32 yd rear loading packer trash truck, using the chains provided on the unit. Apparently, he was standing on the riding step near the packer operating control, when, for some reason, he leaned into the area between the dumpster and the chain while the packing unit was pulling the metal dumpster against the hopper. Employee #1 was caught between the dumpster and chain. His chest was crushed and he was killed.
