Investigation Summary
At 9:30 p.m. on March 16, 2017, an employee was removing the top and bottom hand forged stress relief dies that were installed on a two and a half ton hydraulic forging press. The employee amputated three fingers on the left hand when the upper die of the press suddenly dropped while the employee removed the last bolt with a handheld impact wrench. The employee was hospitalized and treated for his injuries.
