Employee killed in fall to concrete floor

Employee #1 was unloading an air handler from a lift that had been raised to the mezzanine level. He had spread his legs over the space between the lift and the mezzanine floor. He lost his balance and fell 13 ft 8 in. to the lower level concrete floor. Employee #1 sustained a concussion and was killed.
