Investigation Summary
At 5:30 p.m. on March 17, 2017, an employee was loading a truck when he noticed a pallet was broken. The employee exited the truck and was pulling a pallet from a stack to use in repacking merchandise. The employee was struck by a forklift being operated by a coworker and had his left foot and leg run over. The employee was transported by private vehicle to and suffered a broken left ankle as well as a crushed left leg. The employee was hospitalized.
