Employee killed when head crushed by loader lifting arms

Employee #1 was operating a Ford/New Holland loader. He was picking up empty bottles and cans of plastic, glass, and aluminum and dumping them onto a conveyor to be sorted inside. Somehow the employee's head was crushed by the bucket lifting arms. Employee #1 died. When he was found, he was in the loader seat with his seat belt on.
