Investigation Summary
At approximately 9:30 a.m. on May 9, 1994, Employee #1 was working at his machine, which separates foam and aluminum from shredded cars. A conveyor carried the material overhead and dropped it on a shaker table. The employee was under the conveyor when his hand was caught between the conveyor belt and a return roller, which pulled him in up to his head. The employee died 5 days later of cardiopulmonary arrest.
