Investigation Summary
Employee #1 put his head partially through the 8 in. by 19 in. window in an elevator hoistway door to see the location of the elevator car. The elevator was descending and struck his head, killing him. The glass in the window had been partially broken out, allowing Employee #1 to put his head through it.
