Investigation Summary
Between 5:15 a.m. and 5:40 a.m. on December 23, 1998, Employee #1, a temporary worker, was allowed to leave work due to steadily worsening icy road conditions. As she was leaving, Employee #1 slipped and fell on ice in the parking lot. She suffered a fractured skull and died.
