Investigation Summary
A conductor for a compressor fan motor had been incorrectly connected. It should have been connected to a relay that connected it to a step-down transformer to lower the voltage from 288 volts to 24 volts. However, it had apparently been connected to a 288-volt source. The overvoltage had melted the scotch locks on the external wiring for a thermostat, exposing energized conductors. The employee who had wired the improper connection had inadequate electrical training and no supervision from a qualified person. An apartment maintenance employee touched the ends of the thermostat conductors as he was repairing the air-conditioning unit and was electrocuted. (It was not clear from the original form whether the maintenance employee was the one who miswired the unit.)
