Investigation Summary
Employee #1 and coworkers, of Pate Brothers Construction, were installing a sanitary sewer for a new housing development in Irving, TX. They were working in a trench box inside the 22 ft deep trench, but left it to exit the trench. As they did this, the trench caved in, burying Employee #1. He died of asphyxia.
