Investigation Summary
At approximately 9:15 a.m. on May 14, 1993, Employee #1 was unhooking chain slings from a trench shield that had been lowered into an 18 ft deep trench excavation. One shield 8 ft high had been set on the trench bottom, and one 4 ft high had been set on top of it. Employee #1 had just finishing unhooking the chain sling from the 4 ft shield when the earthen side wall above started to give away. Employee #1 ran out of the shield and was buried and killed.
