Investigation Summary
Employee #1 was conducting a framing inspection on a single family residence under construction. He was found in the basement of the building below the fireplace opening, bleeding profusely from the head; he died as a result of his injuries. It is not known from where Employee#1 fell. The fireplace opening is 88 inches below the first floor and there was also an opening to the basement at ground level outside, near the fireplace shaft. There were no guardrails or other warning devices protecting either the first floor opening to the fireplace, or the outside shaft into the basement. There were no eyewitnesses to the accident. Employee #1 was a township inspector and was not covered by Federal OSHA regulations.
